Syntrillo is a virtual care company aimed at reducing the risk of stroke and cardiovascular disease. With SynCare Screen™ we will provide a one-time comprehensive screening to determine your Stroke Risk Score™.
Features for Patients
ONE-TIME COMPREHENSIVE SCREENING
With SynCare Screen™ you'll undergo a comprehensive screening to help identify your risk factors. You will be provided with a smart blood pressure cuff to measure your blood pressure for two to three weeks while we access and review your health records. At the end of your measurement period, you will connect with your care team by video conference, no in-office visits or travel required.
STROKE RISK SCORE™ AND PERSONALIZED CARE GOALS
During your video conference we will provide your Stroke Risk Score™, a proprietary measure of your probability of a stroke in the next 10 years. We will also provide you with some personalized care goals that you can use to manage your cardiovascular health on your own or to determine if you'd like to upgrade to SynCare™ or SynCare+™

SMART TOOLS FOR BETTER HEALTH
The SynCare Screen™ plan includes a smart home blood pressure monitor, sent to your home, which you can keep after your comprehensive screening is completed. The smart blood pressure cuff provides us with the data needed to determine your Stroke Risk Score™.
ONE-TIME, SELF-PAY SERVICE
SynCare Screen™ is a one-time service for . There are no ongoing fees and no long-term contracts. For the one-time fee you will receive a smart blood pressure cuff (which you can keep), your comprehensive screening and determination of your Stroke Risk Score™. The service fee can be applied towards an upgrade to SynCare+™ or SynCare™ within six months of your signing up for SynCare Screen™.
