SynCare™ is a subscription-based telehealth service for monitoring your stroke and cardiovascular health. We use smart devices to monitor daily blood pressure and other activities along with your health history.

Onboarding & Initial Visit

All plans begin with onboarding including patient portal setup, medical history screening, smart device shipping, and an initial telehealth visit with a board-certified neurologist.

Ongoing Monitoring

After onboarding, measure your blood pressure daily using the smart cuff provided. Our 24/7 monitoring will alert you if intervention is needed.
